This position is PRN/As Needed. We are seeking a PRN Program Therapist to support Lincoln Prairie's mission. The Program Therapist performs social service functions to include assisting clients in meeting their bio-psycho-social needs while enabling them to access through therapeutic treatment skills to achieve their optimal level of emotional health, including providing basic consultation, teaching, and specialized clinical social services for patients/families receiving care. The clinical social services provided includes crisis intervention, short-term family therapy, group therapy and discharge planning. The Program Therapist serves as an integral member of the treatment team and is also responsible for case management of patients internal/ external support systems. The primary responsibilities of this position, include: - Identify treatment plan impairments based on assessment findings.
- Provide age appropriate (child/adolescent) clinical interventions (individual, family, group therapy) to patients as determined by assessment.
- Responsible for attending patient staffing for patients in their caseload.
- Responsible for documentation in client medical record following each treatment intervention.
- Responsible for assisting in the discharge planning process for each assigned patient.
- Responsible for case management of caseload and communication with all sources to ensure quality of care.
- Coordinates client staffing and communicates with team on treatment issues.
- Initiates milieu structure within program.
- Responsible for developing age appropriate interventions and special programs for specialized patient populations.
