Job Summary Essential Functions Builds, supports, and maintains effective relationships with internal and external stakeholders and organizations. Represents Banner in an internal/external facing leadership role. Coordinates activities, reviews work, exchanges information, and/or resolves problems in a timely and professional manner. Responsible for operationalizing post-acute alignment strategies, processes, tasks and auditing compliance of data entry and tracking. Submits required reports and recommendations to leadership regarding operations, programs and services. Oversees Care Coordination personnel as assigned. Oversees development for department staff as evidenced by continuing educational opportunities, case study reviews, and mentoring. Provides and/or facilitates Care Coordination and/or other related activities and serves as a resource to other staff members. Works collaboratively with the licensed Care Coordination team to provide feedback and strategize to ensure effective and safe patient discharges. Directs personnel actions including recruiting, new hire actions, interviewing and selection of new staff, salary determinations, training, and personnel evaluations. This position also participates in the development of Care Coordination goals and objectives in accordance with company standards. Ensures all goals and objectives are met timely and effectively. 5. Serves as a resource and provides leadership assistance to achieve outcomes supportive of the organizations strategic plan, as well as optimal operational, financial, and patient/family experience outcomes. Supports and facilitates innovation and change management. Participates in the development, implementation and evaluation of the goals/objectives and process improvement activities within the department and organization. Analyzes data and healthcare trends to gain efficiencies and improve patient outcomes. Participates in the development of the department budget in conjunction with established goals and objectives. This position is accountable for meeting annual budgetary goals. Facilitates communication between members of the healthcare team to promote coordination of care to ensure a positive patient/family experience. Oversees facility patient/family experience results specific to transitional care activities and coaches and manages performance improvement activities to maintain positive patient/family experience.
