This position designs and implements community health interventions in alignment with KP-s enterprise Community Health strategy, ensuring effective integration with KP-s operational and clinical workflows while delivering effective and measurable community and member outcomes in the NCAL. Reporting directly to the NCAL Vice President, Community Health, this position works with the enterprise-wide Community Health leadership team to ensure the KP Enterprise Community Health strategy is implemented effectively in the region, ensuring both national and regional CH goals are achieved. The incumbent will work closely with our Health Care Operations/Care Delivery, Physician Medical Groups, Community Health teams, and other key stakeholders across the organization in order to effectively implement and integrate Community Health strategies. This position is responsible for managing the cohesive quality and timely performance of NCAL Community Health team, while fostering a workplace environment consistent with the values and mission of the Community Health organization.
Essential Responsibilities: - Designs, plans, implements, and ensures effective evaluation of community health interventions that effectively address community health needs. - Ensures community health intervention design, planning, implementation, and evaluation is informed by a wide variety of member and community health intelligence, including epidemiological, health disparities, heath equity, and community needs assessment data. - Works closely with KP-s care delivery and health operations teams to effectively design, plan and integrate interventions into operational and clinical workflows. - Designs, plans, implements, and oversees evaluation strategies to assess community health intervention effectiveness and to inform the continuous improvement of our community health intervention design, planning, and implementation. - Designs, drives, and supports community health initiatives and interventions that concurrently and effectively address individual, population, and community needs. - Ensures community health intervention design, planning, and implementation is aligned to and delivers key operational objectives and clinical outcomes. - Responsible for executing and delivering on goals and objectives associated with the Shared Agenda and Bold Moves, Common Areas of Focus, and other program-wide CH initiatives in region and managing the regional CH team(s). - Develops, evaluates, and maintains effective and efficient partnerships with internal and external stakeholders and vendors that are critical to our Community Health strategy and plan. - Elevates regional/local community needs, best practices and opportunities for scale to the National Leadership team. - Leads, communicates with, and engages regional CH staff and service area teams, creating shared commitment and accountability for effective execution across the region. - Ensures regional Community Health plans and objectives are aligned to the KP system-wide CH strategy. - Establishes planning and prioritization of the CH local priorities aligned to overall CH vision, strategy, and goals. - Partners and engages Regional Health Plan and Regional Leadership to design, plan, implement, and scale KP-s Community Health strategy in alignment with regional needs and opportunities and leveraging KP assets. - Partners with National Sr. Leadership and departments on Community Health strategies, planning, initiatives, and implementation. - Works with Program Office, Medical Group, and Health Plan leaders and other key stakeholders to effectively integrate relevant Community Health programs and initiatives with care delivery. - Establishes and maintains effective CH operations, including metrics that measure the productivity and quality of those operations. - Develops, coordinates and supervises staff within the NCAL CH function. - Integrates National CH strategy and program work into regional planning and work. - Ensures alignment and compliance of regional grant-making approach to the enterprise-wide Community Health strategy and executing to meet regional needs.
Experience: - Minimum ten (10) years of experience and proven expertise in designing, planning, implementing, and evaluating large scale Community Health interventions, including understanding of epidemiology, social determinants of health, health disparities, population health, health equity, community health intervention design, planning, implementation and evaluation, health care operations, and health care delivery. - Minimum ten (10) years of experience with a business or social venture, health system, or health related agency - planning and implementing community development and/or health projects and developing and managing grants. - Minimum seven (7) years of experience in planning, development, and administration of health or social services at local community, county or state level programs. - Minimum of five (5) years of experience in developing and supporting collaborative partnerships - Minimum ten (10) years of progressive leadership experience, with an emphasis on community health and enabling community health outcomes.
Education: - Bachelor-s degree or higher in public health or related field or minimum four (4) years of experience in a directly related field.
Additional Requirements: - Significant experience in designing and implementing process and technology solutions into the care delivery system and into community settings. - Deep understanding of health care operations and clinical workflows. - Clear and effective written and verbal communication skills. - Able to clearly and quickly communicate complex subjects and issues to a broad range of audiences and to tailor communications and presentations to meet the audience needs. - Comfort working in a highly complex organizational setting with a wide range of needs and interests of stakeholders.
