Primary Functions:

• Leads clinical transformation across primary care to accelerate achievement of the organization's strategic vision to fully engage in value-based payment programs
• Supervises assigned or designated clinical directors and other clinical leaders
• Partners with Quality to support execution of programming to ensure success in value-based risk payment contracts
• Supervises initiatives to support new clinician orientation to the CoP
• Assume specific leadership responsibilities to enable standardization of clinical care delivery, clinical excellence, and team models of care across sites
• Work with leadership in specialty practices to better integrate care across the continuum.

Principal Duties and Responsibilities:

• Directly supervises assigned or designated clinicians and promotes their professional development.
• Participates in all phases (e.g. interview, selection, etc.) of clinician recruitment
• Manages physician and advanced practice clinician performance by promoting team based care, role clarity, collaboration, and responsibilities
• Supports PCCM CoP through engaged and proactive communication and project/program prioritization
• Serves as an AOP goal leader for the CoP
• Addresses workplace conflicts guided by our values seeking HR support as needed.
• Actively participates in strategic planning for the CoP
• In collaboration with the CoP Leader and other Clinical Directors, holds clinicians accountable for their individual and collective efforts to achieve success in defined metrics of optimal health, exceptional experience and organizational vitality
• Participates in data review, performance improvement projects, budget preparation, and in efforts to optimize clinical and financial performance
• Supports eBrighthealth ACO clinical and utilization initiatives
• Represents PC CoP as assigned and delegated by the CoP Leader.
• Performs other responsibilities as assigned by the CoP leader

Christiana Care anticipates your duties will involve delivering high quality health care. Your minimum responsibilities are outlined below:

You will be operating under the supervision of the Primary Care and Community Medicine Service Line Leader and administratively responsible to the Chief Clinical Officer or his or her designee. You will participate in the supervision of the office with the Medical Group managers, to include hiring and supervising support staff, participating in recruitment of additional physicians, reviewing all revenue and expense items and participating in practice development and growth. Christiana Care shall provide such facilities equipment and supplies, utilities, janitorial and other support services as Christiana Care shall deem reasonable for the provision of professional and administrative services to patients and administration of the practice. You will maintain a very positive customer service orientation, which will serve as an example for support staff. Physician recognizes that patient satisfaction is extremely important to Christiana Care shall use surveys which it conducts, and other surveys conducted by third party payors to utilizes surveys gauge individual, practice and service line patient satisfaction. In the event that the results of such patient satisfaction surveys are not satisfactory, The PCCM Service Line Leader or their designee to Christiana Care, Christiana Care will work with Physician to remedy such shortcomings. You will participate in marketing activities to promote the Practice and Service Line (e.g., speaking engagements and membership in social, civic, professional and other appropriate organizations). You will submit billing capture data or input forms for services provided by you or under your direction daily for the services provided that day; all charges will be supported by documentation in the medical record within twenty-four (24) hours of the patient visit. Office hours may be conducted during evenings and on weekends to accommodate patient demand. Weekend hours may be required and will be compensated separately.
