Practical Nurse
The Licensed Practical Nurse works under the direction of an RN and reports to the primary care manager. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. Duties may include: 1. Makes observations and thoroughly documents care and the patient's response. Appropriately seeks assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention. 2. The LPN performs, reports, and records assigned nursing care in the Primary Care Clinics, including, but not limited to: a. Oxygen therapy b. Medication and immunization administration c.
