Social Worker (PCMHI)
Primary Care Behavioral Health (PCMHI) program is an outpatient service were patients are seen by appointment and same day with consults typically received from Patient Alliance Care Team (PACT) members. PRIMARY CARE MENTAL HEALTH INTEGRATION (PCMHI) SOCIAL WORKER - Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Incumbent must possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups. - Establishes and maintains effective therapeutic relationships with veterans and their families.
