Program Director for I-CAN
OHSU School of Nursing has been nationally recognized by the Robert Wood Johnson Foundation Campaign for Action as one of six schools with a promising model for educating nurses for population health practice and leadership. I-CAN is an innovative interprofessional education and collaborative practice program that provides community-based, interprofessional care coordination to socially isolated and vulnerable people and populations. It demonstrates that providing patient-centered health care focused on social determinants of health (the conditions in which people are born, grow, live, work, and age) can be more effective than only focusing  on disease management.
