This position supports the BHIP Home Team needs; including CDA and Wenatchee CBOCs. The RN facilitates and monitors direct patient care to Veterans. The BHS RN Care Manager will work in collaboration with their assigned interdisciplinary team members to assess and plan delivery of care or arrange services in their local community through a community provider. The nurse will work directly with the team(s) psychiatric prescribers and other facility medical staff to ensure comprehensive care. Duties of the position include, but are not limited to the following: Provide direct patient care in support BHIP psychiatry staff Assist with TMH provision of care. Direct interventions with client and the family, or care management of other resources for the client. Crisis intervention/Risk assessment Psychiatric assessment and community referral. Triage and assessment; including vital signs, reported symptoms, and question responses. Contact of patients with high risk responses, significant changes in condition, or changes in other specific elements. Identification and intervention for potential exacerbations or complications to facilitate: Triage of calls and concerns of patient/family, resolve those within scope of practice, and route others to mental health care staff or other services as indicated. Communication and data exchange with community-based providers when there is co-managed care. Provide psychosocial support to patients. Provide interdisciplinary consultation and interventions such as with other non-institutional care programs and venues, mental health, social work, pharmacy, nutrition, etc. Identify patients' knowledge, health factors, skills, and behaviors that support self-management and identify gaps therein. Provide health care coaching, patient education, and motivational counseling. Communicate with provider regarding progress to goals; patterns or trends of data, symptoms or findings; concerning findings; need for provider assessment and/or interventions. Facilitate and communicate treatment changes directed by the provider and provide follow up evaluation of the patient after the changes are implemented. Adhering to program requirements for documentation of patients and progress following admission to a BHIP/Home Team.
