The Atlanta VA HealthCare System is recruiting for several full-time RN Care Manager's to support the Home Based Primary Care Program (under Geriatrics, Extended Care and Rehabilitation Service) in the following Community Based Outpatient Clinics (Cobb County, Decatur, and Pike). The Atlanta Veterans Affairs HealthCare System is recruiting for several full-time Registered Nurses for the Home Based Primary Care Program (under Geriatrics, Extended Care and Rehabilitation Service) in the following CBOCS, to include (Cobb County, Decatur, and Pike County). The Registered Nurse will function as the Care Manager, linking the patient and the caregiver with various health services that are offered by the inter-disciplinary team and the health care system. The Registered Nurse utilizes knowledge of nursing practice to provide holistic care to Veterans and their caregivers in their homes. The nurse is responsible for initial and continued assessment of the veteran and collaborates with HBPC team members. The individual must have excellent clinical skills, be self-directed, organized and a good communicator. The individual demonstrates exemplary customer service. The Registered Nurse provides appropriate care and emotional support to the terminally ill patient, monitors new medication regimes and effects, educates and provides follow-up treatment for patients, documents assessment data, and provides case finding and case management. The majority of time will be spent in the community caring for patients in their homes; the Registered Nurse will be responsible for traveling within a 50 miles radius for patient home visits. The Registered Nurse specific duties include, but are not limited to: Functioning as Care Manager for assigned patients. Performing nursing assessments and reassessments of veterans and caregivers. Delivering nursing care in the home. Providing patient education to the veteran and caregiver. Reporting findings needing medical attention to HBPC providers. Functioning within nursing scope of practice and functional statement. Participating in HBPC Performance Improvement and accreditation processes. Supporting veterans, caregivers, and their significant others. Educating on home safety and symptom control including pain management. Providing and instructing on prevention and management of wounds and other conditions. Performs other duties as assigned my management.
