About NACDD The National Association of Chronic Disease Directors (NACDD) is seeking a full-time staff person to serve on the Diabetes Technical Assistance and Support team (TA and Support Team) as a Program Evaluator. NACDD is a Member-based not-for-profit association that improves the health of the public by strengthening state-based leadership and expertise for chronic disease prevention and management. In collaboration with the Centers for Disease Control and Prevention (CDC), the TA and Support Team serves all 50 State Health Departments, the District of Columbia, and national, state, and local partners to scale and sustain the National Diabetes Prevention Program (National DPP) lifestyle change program, diabetes self-management education and support (DSMES)
