The Stroke Program Coordinator is fundamental to the development, implementation, and evaluation of the stroke program directed towards the provision of quality services for patients who are experiencing stroke from a multi-disciplinary perspective throughout the continuum of care. This individual works collaboratively with the Stroke Program Medical and Surgical Directors in a mutually supportive relationship which is essential to the success of the program. The Stroke Program Coordinator is an expert nurse who provides leadership for the organization of services and systems necessary for a collaborative approach to stroke care. The Stroke Program Coordinator will assume the day-to-day responsibilities of process and performance improvement activities as they relate to nursing, and ancillary personnel and assist in carrying out the same functions for all staff. As a Stroke Program Coordinator, you need to know how to:
- Plan, implement and evaluate clinical programs to achieve high quality, patient-focused outcomes which advance patient and family care and nursing practice in stroke care.
- Stroke Program development, quality assessment, improvement, and inter/intradisciplinary communications.
- Work collaboratively with all hospital and clinic personnel on shared system and performance improvement projects.
- Serve as a coordinator for assigned program.
- Discuss program with physicians, nurses and Cardiovascular leadership team and concurrently review that all care meets evidence-based guidelines.
- Provide and/or oversee required education to patients and family members during the hospital stay and serve as a resource and contact for patients and their families after they are discharged.
- Maintain and/or analyze data collected on assigned program. Serve as the leader and subject matter expert for the program. Create and maintain a Quality Assurance program and coordinate all program certification/accreditation activities.
- Promote program to internal and external audiences.
- Promote mission, vision, and values of SCL Health, and abide by service behavior standards.
Your experience.

- Associateâ™s Degree or higher, required
- Current Colorado licensure, registry, or certification in a clinical field, required
- Current BLS certification, required
- Minimum of three (3) years of relevant clinical experience (i.e. Cath Lab Tech, RN, etc.), required
- Bachelorâ™s Degree in Nursing (BSN), preferred
- Current Colorado RN license or ability to obtain, preferred
- Current ACLS certification, preferred
