The CPP functions at the highest level of clinical practice and serves as an advanced practice provider working autonomously with prescribing privileges as outlined in the individual's scope of practice. The CPP provider delivers evidence-based comprehensive medication management (CMM) through direct patient care and as part of the HBPC interdisciplinary teams. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. Grade Determinations, GS-13: Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level and the following knowledge, skills and abilities (KSAs): 1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. ["Major Duties: Responsible for all assigned HBPC patients; must work independently and effectively to assure appropriate, accurate drug reviews are completed. Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate recommendations for alternatives to current treatment plans as needed. Performs initial and periodic assessment of assigned HBPC patients' medication therapy, monitors as needed, and makes recommendations or changes (within scope of clinical privileges) as necessary. Timeliness of reviews will be determined by JCAHO, HBPC, and/or other regulatory requirements. Performs comprehensive medication management (CMM), including the initiation, modification, and discontinuation of medication therapy, through interaction with both patients and fellow health care team members and identify those patients who may benefit from Comprehensive Medication Management (CMM) through population management reports and during team huddles. Follows up via face-to-face, telephone, VA Video Connect (VVC), eConsult, secure messaging and other modalities to provide direct patient care. adjudicates HBPC prior-authorization drug requests (PADRs) and evaluates restricted medication requests for appropriateness and compliance with established criteria where applicable. Serves as a subject matter expert fr HBPC assigned committees, workgroups, or interdisciplinary teams. Participates in self-development activities to maintain and strengthen professional competence in HBPC. Supports and promotes compliance with established drug therapy policies, formulary management and criteria or prior authorization requirements that have been approved by national, VISN, or local pharmacy benefits management (PBM) and focuses on quality patient care in a cost-effective manner. Participates and facilitates departmental and organizational compliance with The Joint Commission (TJC), Office of General Counsel (OIG), and other applicable regulatory and accreditation regulations and guidelines regarding patient safety and care. Supports efforts to improve performance metrics specific to HBPC. Participates in P&T committee functions and assignments as required. Maintains effective interdepartmental relationships by committee membership, barrier resolution, active problem resolution skills, and close cooperation with other services in accomplishing the medical center mission and goals. Provides formal and informal education and in-services to pharmacy, medical, nursing, and other staff as requested. Serves as preceptor for pharmacy residents and students in accordance with established policies. Functions: Functions as part of the interdisciplinary team to provide CMM for HBPC which may include, but is not limited to: Prescribes, monitors, and adjusts medications related to HBPC. Obtains pertinent medical history and obtains accurate medication history from patient and via chart review. Orders, monitors, and interprets applicable labs; implements follow-up on treatment actions as supported by lab interpretations. Queries and documents PDMP findings, as applicable. Provides patient and/or caregiver instructions and education. Ensures referral of complex or unstable patients to higher level care, when needed Addresses other care needs and refers patients to other services for other needed care, as deemed appropriate Communicates with team for next steps and coordination of care, when necessary Accepts urgent or same day patient visits including, but not limited to, patient medication review for polypharmacy, recent hospital or emergency department discharges, and co-managed care patients.
