A Licensed Vocational Nurse who supports population health initiatives in the Care Integration department using different modalities across the healthcare system, which includes hospitals, clinics, and virtual care. This position works under the direct supervision of the registered nurse, advanced practice registered nurse, physician assistant, physician, dentist, or podiatrist. Uses the nursing process to ensure physical, social, psychological and spiritual needs are met, including post-discharge education. Responsible for assisting in the determination of predictable healthcare needs of clients within structured healthcare settings who are experiencing common, well-defined health problems with predictable outcomes. Utilizes a systematic approach to provide individualized, goal-directed nursing care, including a focus on chronic disease management (diabetes, hypertension, hyperlipidemia, etc.) and proper transition of care. Coordinates care between members of the healthcare team to optimize the patient/client's understanding of their condition and care requirements within the LVN scope of practice.

Minimum Qualifications:
Graduate of an accredited school of nursing.

Current licensure as a Licensed Vocational Nurse in the state of Texas.

Experience in area of specialty preferred.

Bilingual Spanish-speaking preferred.
