ResponsibilitiesThe Health Risk Assessment Program supports UCLA primary care in ongoing proactive, complex care management of high risk patients to provide a comprehensive management approach to primary care. The Nurse Practitioner (NP) will be an integral practitioner of the program, which will include use of telemedicine and innovative approaches to patient care. The program will include in-home visits to high risk patients based upon clinical needs, as per the direction of the UCLA primary care team.
