The social worker collaborates with the multidisciplinary team, lending insight and expertise regarding the psychosocial and post-services needs of patients, offering recommendations for appropriate discharge planning and general disposition following treatment. Acts as a liaison between hospital and family when patient is transferred to other facilities.
