PRIMARY FUNCTION:The Logistics Coordinator is primarily responsible for the facilitation of hospital care at home patient care visits in the patient's home, hospital care at home patient admissions, and discharges, by scheduling and coordinating the primary in-home clinicians (RN and Paramedics) and external contracted suppliers and providers. The Logistics Coordinator leverages a variety of tools for coordination including telephone, computer, and software.
