Overview Atlantic Health System, is seeking an In-patient PD Care Manager for their Newton Medical Center. Responsibilities Essential Functions:Job Function: 01. Responsible for facilitating and coordinating the care delivered to an assigned group of patients through multidisciplinary and patient/family collaboration to ensure that quality and cost effective outcomes are delivered within appropriate length of stay (LOS) parameters. Coordination involves assessment, planning support and evaluation of patient care and related outcomes. Activities to be performed include patient/family interviews, clinical resource management, utilization review, coordination of discharge planning activities and assurance of comprehensive clinical documentation for the age-sp
