Facility Program Manager (Hematology/Oncology) Pharmacist
The Facility Program Manager (Hematology/Oncology) Pharmacist provides comprehensive medication management for hematology/oncology patients, including both inpatient and outpatient specialty clinic settings. This position provides support to oncology pharmacy services through clinical and distributive services, direct patient care and management, development, implementation and maintenance of order sets, protocols, and clinical algorithms. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Preferred Experience: Successful completion of PGY-1 pharmacy residency training, successful completion of PGY-2 pharmacy residency training in oncology, Board Certified Oncology Pharmacist
