SUMMARY OF POSITION:
Community Paramedicine Clinicians will work with patients in and out of hospital environment providing clinical care to patients who have been discharged from the hospital. These clinicians will also work with the various service lines, including physicians, nurses, care coordinators and other members of the health system to with various transitional care needs and to mitigate preventable readmissions. These clinicians will also provide real time feedback to physician and nursing staff on clinical findings of patients who might need follow up care.
MINIMUM QUALIFICATION(S):
High School Diploma required
Graduate of an approved Paramedic training program required.
Emergency Medical Technician - Paramedic (EMT-P) certification within the Commonwealth of PA or Pre-Hospital Health Professional credential required.
Two (2) years of experience with Life Lion EMS as a Paramedic (or)1 year of experience with Life Lion EMS as a paramedic and 1 year previous supervisory EMS experience (or) PHRN (Pre-Hospital Registered Nurse)
Three (3) years total of pre-hospital experience required
Basic Life Support certification, Advanced Cardiac Life Support certification, Pediatric Advanced Life Support certification, EVOC certificate, and NIMS (100, 200, 700 & 800) are required
A valid PA Driver's License with at least a 2 year driving history before beginning employment is required
Completion of approved Emergency Vehicle Operations Training program PA DOH EMSVO NIMS (100, 200, 700 & 800)
PA Hazmat awareness training is required within 30 days of employment.
Must obtain Medical Command within 90 days of start date.
