Physician (Geriatric/Transition of Care Medical Director) with EDRP
The South Texas VA Health Care System is currently seeking a Physician (Geriatric/Transition of Care Medical Director) to provide high quality patient centered care to Veterans under the difference services such as GEC community programs, GEC transitions of care program, Hospital in Home program Community Living Centers, Home Base Primary Care, GEM clinic and Palliative/Hospice Care services.
