Job DescriptionJOB SUMMARY: In collaboration with a multidisciplinary team, the social worker is responsible for completing psychosocial assessments, assisting with treatment planning, monitoring patient progress, facilitating patient and family meetings, and coordinating and implementing discharge and aftercare services for patients in the behavioral health inpatient unit.
