JOB SUMMARYThe Service Linkage Worker (SLW) provides services within a broad range of focus areas to assist patients in accessing and adhering to care. Services provided may include HIV testing and counseling, mental health and substance abuse patient assessments, assisting to reduce access and adherence barriers, follow-up to ensure referrals are completed, patient navigation assistance, and coordination with case managers and other clinic staff.
