IU Health Physicians is seeking a Nurse Practitioner or Physician Assistant to join our Primary Care team at IU Health Connected Care- Indianapolis! Connected Care provides comprehensive primary care for adults on Medicare. By providing longer visit times, improved coordination of care with specialists, and more frequent appointments, we strive to improve the experience and outcomes for patients dealing with multiple chronic and/or social health issues.
