PRIMARY RESPONSIBILITIESActs as an operational liaison between select out-of-scope SSM Health teams; provide coordination across the care continuum with focus on case management.Manages internal and external stakeholder relationships to communicate progress, risks, issues, and mitigation to Care Transformation (CT).
