Job SummaryThe Director of Care Integration is an innovative and experienced leader responsible for the operational, financial, programs and personnel activities for the assigned population health initiatives and/or departments. This leader works closely with the Admin Director to develop and establish long-term strategic plans for the department integrating the population health model of care, supported by data science that encompasses continuum of care coordination, social determinants of health, community collaboration and outreach.
