Under the direction of the Manager, Quality and Care Coordination, this position is responsible for making initial contact, and actively managing patients who have utilized emergency department services in conjunction with the practice care coordination team, physicians and patient care teams. Job Duties: Assists onsite clinical care teams in providing follow up contact with patients to ensure compliance with recommendations, referrals to specialists, hospitalizations, ER visits, ancillary testing, and other services. Documents patient contacts, education and communication with other providers regarding patient care in the EMR. Collaborates with physician leadership to define quality measures and outcomes, reporting processes, and develops protocols and point of care reminders using nationally recognized evidence-based care measures and outcomes. Collaborates with consulting physicians, hospitals, ER, and other healthcare resources to clarify roles and develop effective and timely communication between primary care provider and these entities. Utilizes and interprets patient data from EMR, payer information and other resources for quality improvement activities, identification of high risk patients, gaps in care, preventive health needs, utilization review/management and other pertinent patient related information. Effectively uses registries and other population health management tools provided by payers. Oversees reporting to ensure adherence.
