Social Worker
OverviewThe Social Worker is responsible for the provision of case management and community liaison services in WakeMed Physician Practices - Pediatrics. This includes: assessing safety and social needs of patients during clinic visits; supporting integration of patients with needed medical, social, and developmental services/resources; supporting families of patients in connecting with psychosocial services which directly or indirectly impact the patient's well-being; being a liaison for community-based programs and services, including schools; and, providing direct intervention for social needs while patients are in the practice.
