SUMMARY OF POSITION:

Provide targeted case management services to child and adult mental health consumers with serious and persistent mental illness. Services offered include functional assessment, service planning, support, network building, linkage of services, monitoring, outreach and problem-solving. The goals of these services is to improve the quality of life, reduce frequency and length of psychiatric hospitalizations and incarcerations, increase independent living ability and strengthen social supports. 

MINIMUM QUALIFICATION(S):

• Bachelor's Degree 
• Basic Life Support (BLS) required prior to completion of orientation
  
• Valid driver's license required.
