Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (1-5 years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• The ANP in Home Based Primary Care (HBPC) collaborates within a Patient Aligned Care Team (PACT) to provide clinical care to patients through an outpatient - home-based system
• The ANP functions as the case manager for a complex Veteran population - providing hands-on and virtual patient care - and assumes primary medical responsibility for the patients assigned to their PACT team
• The ANP will work with an Interdisciplinary Team to develop a personalized care plan for each patient
• The ANP will provide needed updates as patient physical or Psychosocial condition changes
• Duties include but are not limited to: Functions within an interdisciplinary team that delivers care in the home to Veterans who are unable to come to the medical center for routine clinic appointments
• Assumes primary medical responsibility for the HBPC patients in their case load - identifying medical problems and defining their medical management
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with other interdisciplinary team members
• Orders and interprets labs - tests - and diagnostic studies
• Provides timely follow-up on results - including completion of view alerts
• Performs and records history and physical examinations and records data from clinic visits
• Enters progress notes in computerized medical record and completes workload documentation in a timely manner
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
• Consults other specialties as appropriate - according to service agreements
