OverviewThe Medical Assistant, Population Health understands the needs of the practice and supports the mission and values. Medical Assistant Population Health (MAPH) works interdependently with other medical professionals to help manage the health and wellness of patients. MAPH will assist in the care coordination of patients with chronic diseases, those who have been recently hospitalized or discharged from the Emergency Department, and other patient populations as appropriate. Uses motivational interviewing to discuss services due with patients based on standing orders and quality measures guidelines. Follows up on previous orders and consults with primary care providers.
