DescriptionDEPARTMENT: Community HealthPOSITION SUMMARY: The goal of the Adolescent Behavioral Health Crisis Counselor is to provide behavioral health assessment, care management and interventions for adolescent patients presenting in behavioral health crisis in the CSV emergency department and/or CSV clinical departments. Provides behavioral health assessment, crisis interventions, and case management for adolescent patients experiencing a behavioral health crisis. Works collaboratively with medical providers, psychiatric providers, and other care team members, to ensure the patient is referred to the appropriate level of care. Provides clinical case management to transition the patient to the appropriate community se
