Orlando Health Arnold Palmer Hospital seeks a dedicated physician to join our innovative Transition Care Program within the Orlando Health Medical Group. This specialized role focuses on providing coordinated care to young adults with medical complexity as they transition from pediatric to adult healthcare systems. This position is housed under our pediatric specialty service line and offers a unique opportunity to work at the intersection of pediatric and adult medicine.
Key Responsibilities
Provide comprehensive medical care to adolescents and young adults (typically ages 16-26) with complex medical conditions
Coordinate seamless transitions from pediatric specialists to adult care providers
Manage a diverse caseload of patients with chronic conditions requiring ongoing medical management
Collaborate closely with pediatric specialists, adult subspecialists, and multidisciplinary care teams
Develop individualized transition care plans that address medical, psychosocial, and developmental needs
Serve as primary care provider during the transition period, offering continuity and stability
Provide both outpatient clinic care and inpatient hospital coverage as needed
Participate in care coordination meetings and family conferences
Maintain detailed documentation and communication with referring providers

Qualifications:
Education: M.D., D.O.
Completion of an ACGME or AOA Pediatric Residency and Fellowship in Adolescent Medicine
Board Certified or Board Eligible by the American Board of Internal Medicine or Pediatrics
Will Consider Physicians Boarded in Family Medicine or with Experience in this Area
Unrestricted FL medical license (or the ability to obtain one) and DEA
