Come join our team.Job Summary: The Faculty Clinical Pharmacist works as part of a multidisciplinary team in the Family Medicine Residency Clinic. The Pharmacist will provide clinical pharmaceutical services to patients of all ages and participate in Resident education; as well as provide pharmacologic counseling and education to patients to include diabetes treatment, coumadin dosing, and other chronic disease management.Essential Functions & Responsibilities:Provides pharmacologic counseling and education to patients, on-site consultations with faculty/Residents, and provides/receives warm hand-offs to/from other faculty and Residents.
