Position Highlights
•     Full-time, outpatient primary care with 4- or 5-day work week options
•     Quickly build your practice with an established patient base
•     Provide comprehensive care for patients of all ages, including acute and chronic disease management
•     Opportunity to perform screening colonoscopies
•     Ability to perform small office procedures (laceration repair, wart removal, I&D, etc.)
•     Shared after-hours call supported by a nurse triage system
•     Access to on-site and nearby services including: Outpatient lab, Pharmacy, Radiology, Physical and Occupational therapy 
•     Opportunity to teach and mentor residents through the Family Medicine Residency Program at IU Health Ball Memorial Hospital
•     Strong team culture focused on quality, patient experience, and provider support
•     Community-based practice with the backing of a large, integrated health system

Role Expectations
•     Establish and manage a patient panel drawn from Hartford City and surrounding areas
•     Use measurable quality-based data to improve patient outcomes and support population health
•     Deliver evidence-based, compassionate, and equitable care
•     Contribute to IU Health’s mission to make Indiana one of the healthiest states in the nation

Ideal Candidate
•     MD or DO with board certification or eligibility in Family Medicine or Internal Medicine
•     Passion for rural health and community engagement
•     Interest in teaching and mentoring learners
•     Commitment to high-quality, patient-centered care
