Overview

• Collaborating with the clinical team to develop treatment plans addressing psychosocial needs
• Communicating complex patient/family needs and modeling caring practices
• Assessing psychosocial risk factors, prior functioning, support systems, and reactions to illness
• Counseling patients and families on emotional, social, and financial impacts of health issues
• Mobilizing community and family resources to meet identified needs
• Providing mental health assessments upon physician referral
• Facilitating complex discharge planning to ensure safe transitions of care
• Serving as the point person for cases involving abuse, neglect, domestic violence, guardianship, foster care, and adoption
• Acting as liaison with child and adult protective services and community agencies
• Supporting crisis intervention and de-escalation techniques, including managing potentially dangerous situations
• Utilizing evidence-based psychosocial assessments and crisis intervention strategies
• Leading decision-making conversations about goals of care and discharge planning
• Adapting services to meet the needs of diverse populations across cultural, geographic, and financial backgrounds

• Education: Master's Degree in Social Work (MSW) required.
• Licensure: Permanent LCSW licensure required.
• Certifications: Crisis Prevention Intervention (CPI) or equivalent de-escalation training within 6 months of hire.
• Experience: Proven experience in crisis intervention, mental health, or substance use disorders, preferably in hospital or pediatric settings.
• Knowledge & Skills:
  • Evidence-based psychosocial assessment and short-term therapeutic counseling
  • Knowledge of disease processes and human behavior related to health social needs
  • Complex discharge planning and coordination skills
  • Familiarity with legal mandates for child and adult protective services
  • Ability to work effectively with multidisciplinary teams and community agencies
  • Computer literacy and proficiency in electronic medical record documentation
  • Experience serving geographically, culturally, and financially diverse populations
• Additional:

  • May be scheduled for on-call shifts and holiday rotations
