Home Based Primary Care (HBPC) is a home care program designed to meet the longitudinal, primary care needs of an aging veteran population with complex, chronic, disabling disease. Will be responsible for directing the clinical and administrative services offered by the program to ensure that the program is in compliance with local and Veterans Health Administration (VHA) standards and policies as well as accreditation standards for home care organizations. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
