Provides case management for patients with chronic kidney disease.  Collaborates with internal and external providers to coordinate the plan of care and provide expertise in chronic kidney disease treatment including dialysis.  Accountable for ensuring effective coordination of patients with chronic kidney disease.Essential Responsibilities:Acts as case manager for the care and follow up of patients in the specialty clinic and outpatient dialysis centers. Arranges and monitors follow-up appointments. Encourages members to follow prescribed course of care (e.g., drug therapy, physical therapy). Makes referrals to appropriate community/contracted services.
