Salary Range:
We are seeking a highly motivated and compassionate Advanced Practice Provider (APP) to join our multidisciplinary Post-Transplant team. The APP will play a vital role in the ongoing care and management of patients following solid organ transplantation (e.g., kidney). This provider will work collaboratively with transplant physicians, surgeons, nurses, pharmacists, and other care team members to ensure the best possible outcomes for post-transplant patients.
Salary Range: $124,693 - $145,475
Key Responsibilities:
Provide comprehensive post-transplant care, including assessment, diagnosis, treatment planning, and follow-up management.
Collaborate with transplant coordinators, social workers, dietitians, and other team members to support patients' clinical and psychosocial needs.
Educate patients and families on post-transplant care, including medication adherence, lifestyle changes, and signs of complications.
